Medvedev made his ATP main draw debut in the 2015 Kremlin Cup doubles tournament. He won his first singles match at the Ricoh Open in 2016.

Medvedev earned his first ATP Tour championships in Sydney and Winston-Salem, as well as his first ATP 500 title in Tokyo, in 2018. In 2019, he made his top ten debuts following Wimbledon and reached six straight tournament finals, including the US Open.
